What should be the subsequent treatment plan for sarcoma patients with lung metastasis to control the disease?

 Case Analysis

. In March 2014, Ms. G discovered a painless tumor approximately 2 centimeters in size on her right lower gum and underwent immediate surgical removal.

However, the pathology report indicated a soft tissue sarcoma that had invaded bone tissue, favoring a mesenchymal chondrosarcoma or extraosseous Ewing's sarcoma. No additional adjuvant therapy was administered thereafter.

During this period, she lived a stable life for four years. However, in March 2018, a follow-up examination revealed multiple metastases in both lungs. A biopsy of the left lung confirmed sarcoma metastasis. Due to the presence of lung metastases, the stage was classified as IV. The patient did not actively seek treatment and only underwent radiofrequency ablation for the lung metastases in December 2018. However, a follow-up examination after radiofrequency ablation showed that the lung lesions had slightly shrunk compared to before, with the largest lesion still measuring 1.9 centimeters.

Due to the lack of significant early treatment effects, Ms. G was somewhat concerned about the potential emergence of more metastases in the future. Therefore, it was decided to adopt NKT cell immunotherapy in the hope of controlling the lung metastases.

In April 2019, Ms. G received the first course of NKT immune cell infusion, with a regimen of one course per month. As of January 2021, 19 courses of infusion had been completed.

 

Treatment results

Through timely, continuous, and systematic individualized NKT cell immunotherapy, as well as four follow-up treatments,

 

Imaging Aspects

After the patient underwent NKT cell immunotherapy, the largest pulmonary metastatic lesion had disappeared by January 2021, compared to its previous size of 1.5cm*1.2cm in March 2019. The left lower lung nodule had slightly shrunk compared to before. No new lesions were observed in other organs such as the liver.

Blood and quality of life aspects

From April 2019 to June 2020, all tumor markers tested were within the normal range and remained stable.The quality of life has been greatly improved, with a weight gain of 1-3kg, good sleep and diet, and a quality of life score of 92 points.

 

Conclusion

After 19 courses of NKT cell immunotherapy, Ms. G's largest pulmonary metastatic lesion has essentially disappeared, while the remaining smaller metastatic lesions have either shrunk or remained unchanged. No new signs of disease progression have been observed in other organs. Her progression-free survival (PFS) has been as long as 21 months, and she continues to undergo treatment and follow-up.
